Am I the only one who feels that this was a rather sad episode, particularly how Homer basically dismisses Grampa’s calls to stop the car so he could urinate? I thought Homer’s behavior there was pretty monstrous, despite him being a complete idiot.
In regard to the above screenshot, I love the look on everyone’s faces — the Asian tourist and others smiling delightfully, Bart all wide-eyed in what one could interpret as disbelief, and Homer’s “whatever” expression.

Agreed. Homer’s behavior goes way beyond ‘local oaf’ or even jerkass. He’s just despicable. I remember this ep as the first time The Simpsons left me absolutely cold.
No, you’re not the only one who thought Homer was heartless in this episode. To make matters worse, the whole kidney blowout thing isn’t real. Your bladder is the affected organ in situations like that, and I think once it blows and the urine causes sepsis in your body, you’re a goner.
On the plus side, I did like the parts where Homer mistakes a taffy shop for a real ship, the Sea Captain not painting over the Honeybunch name on the boat, and the people on The Ship of Lost Souls (especially the Peter Lorre caricature and the Frenchman who stole his accordion [concertina, actually. Accordions are boxy, worn on the body, and have a keyboard] from a blind monkey, yet thinks Homer abandoning his father is horrible).
This seems to be one of those episodes that a lot of people really dislike, mainly on the basis of Homer’s treatment of Grampa. To be honest, I’ve always liked this episode – the plot is so utterly absurd that Homer’s behaviour doesn’t bother me. (And besides, are we all forgetting Abe cackling gleefully over his son’s near-deathbed in “Homer’s Triple Bypass”?)
The difference between what Grampa did and what Homer did is that it was a quick scene and it was written with more subtlety and humor.
It’s not really the same, because I have always assumed Homer cared about Abe a little more than Abe cared about him, and because in “Homer’s Triple Bypass” it was just a quick joke, whereas the kidney thing was the entire basis of a plot. Also, Grampa once had a heart attack in “Oh Brother Where Art Thou”, and he badly treated his son (“Ummm, interesting theory”).
There’s nothing in the real Simpsons seasons that suggest Homer would have let Grampa die if he had a way to prevent it, which is why so many people were very upset about this episode. Homer being scared yet allowing the operation? Highly plausible, and a rich source for comedy. Homer running away twice and letting Abe die? That’s Jerkass Homer for you.
In a proper Simpsons episode, the Grampa loses a kidney -> operating needed -> operation made part of the episode would have lasted one act. Maybe one and a half, but they would have written a more dense story. Here, it just slows down things to a terrible pace.
